Heater core leaking (most likely) or the ACC recir flap motor is broken. Did you notice a sweet smell (coolant) at all when the issue was occuring? Was it just the windshield? If yes to both, its definitely the heater core.
Search for Bars Leaks and you'll find many many opinions of the stuff. I myself will never use it, since the PO of my car did and it clogged my heater and did a couple of other things to the system. When your HC starts leaking that badly, you're going to have to replace it. Period. Additives are just bandaids and will serve only to cover up the issue, but eventually the thing will need to be replaced. Others swear by the stuff and use it regularly.
If you didn't notice a smell and all your windows (not just the windshield) were fogging up, it could be the ACC flap motor. But its probably still the heater core. I'd check the operation of the flap before paying a lot to have the job done, or spending a lot of time doing it myself.
