As well as the things you mention, it still might be worth trying another DI casette if you can borrow one, because when this went on my old '93 car the symptoms were much the same. And I'm sure you have tried new plugs (and checked that they are the correct ones).

We don't seem to read much here about injector problems. But maybe I'm just suspicious of ignition systems – on the first of my ancient Saab 96 V4s, in about 1975, a similar problem was traced to the distributor base moving and upsetting the advance/retard setting. I drilled the side of the casing and put in a set screw to hold the base securely.
