Although it is possible for one to fail without warning, my experience is that they give warning before failure. The typical warning is a squeal on startup that quickly stops. Over time, the squeal lasts longer and longer. I ran one about 2 weeks before replacing it.
It's pretty easy to monitor. The best way is to remove the wheel arch, but I've had good luck with a flashlight and an inspection mirror. As yaofeng says, visual inspection works. The other thing to do is to make paint marks across the face of the pulley, crossing the rubber part. Then periodically take a peek at the marks when the engine is off. If the lines are straight, the pulley is OK. If they start to drift off, then the inner and outer parts are moving at different speeds, and the pulley is in the process of failing.
Given the cost of the pulley (about $120 last I checked), that it usually gives warning, and that it can be easily inspected, I wouldn't replace it without reason.
