When you did the lower mounts, did you replace the bushings in the upper tension rod ('dogbone')? How do the bushings there look? That tension rod takes most of the force under engine accleration/deceleration. If those bushings are shot, then the lower mounts will get a lot of movement, stress, and then fail.
I'd also look into the health of the transmission mount.
