Upon inspection there seem to be two problems. One, I used too short a bolt on the uppermost tensioner bracket mounting point (must have been tired or lazy that day -- not characteristic of me). That one appears only to have had maybe 1.5 threads worth of purchase.
I have a similar feeling about the one below that, but can't be sure because the bolt is now missing.
The loss of both upper mounting bracket bolts caused the tensioner assembly to detach from the head, and allowed the belt to fall off.
I'm hopeful the threads can be retapped and longer bolts inserted. Anyone know for sure the proper size and pitch? These are both the rather ubiquitous 10mm heads in use lots of other places -- I may even have the right spares in my stash someplace.
I'm hopeful that it's just a matter of replacing the belt. I will probably take this opportunity to flush the old coolant while I'm at it, provided that car doesn't exhibit signs of a failed head gasket.
