If you look at the throttle body and then at the elbow, you will see a groove in the throttle body and a raised ring molded into the elbow. If you don't push the elbow all the way home, or push it too far onto the throttle body, the ring does not fit into the groove properly and the hose blows off. Once you are sure you have the ring in the groove, make sure the hose clamp is in the right spot and tighten it down firmly. You should not be able to rotate the hose.
