Lack of power at low RPM and check engine light

My car is a 1996 CSE with 150k miles. Ever since I had the car last year when it was 100k miles, it lacked low end power most of the time. To start from a stand still, I need to rev the engine and release the clutch very slowly to avoid bad engine shaking.
This summer, I replaced all vacuum hoses, which made things instantly better -- the car would move from stand still easily with a little gas. So I thought I fixed the problem by getting rid of vacuum leak. But only a few weeks later, it went back to the state of lacking low end power again.
I suspected fuel filter, but the car runs much better at higher RPM. My understanding is that a clogged fuel filter will affect higher RPM more. I also noticed the car is much harder to get boost than it did last year. I used to be able to get almost full boost by simply revving the engine in neutral, but now it only gets boost under good load. If I press it enough, it does get full boost, though.
Also occasionally the car gets check engine light, which turns out to be 'multi cylinder misfire'. The DI cassett is relatively new, replaced 30k miles ago. The spark plugs are new. The hooter valve is new. The air filter is new. So I am really puzzled. What could be the cause of my issue? Could it be the fuel pressure regulator? I still think it's something related to vacuum, as replacing all vacuum hoses this summer made things instantly better. But then why it went back again?
