Though bolted tight, there is a rubber isolation layer in the pulley that can get old and weak, leading to major wobble. The most common (speaking based on anecdotal evidence from this forum only) is to have the inner portion of the crank "skid" along the rubber isolator, leading to nasty howling. But I've personally replaced one crank pulley that failed into serious wobble mode and took out the belt. Forgot about that one, or I'd have suggested it too.
A little play (talking a max of a millimeter or so) in the idlers shouldn't cause you nightmares, so if you think the crank pulley is gone you might get away without an idler replacement. Good thing if so, because the crank pulley is pretty pricey on its own.
