Same thing happened in my 94 Aero. Original trans failed at ~185K, went the rebuilt route. No idea on the mileage on that one, but the internal gear failed after 8mos (out of warranty obviously). I then drove for almost 2yrs and just recently replaced the faulty rebuilt unit with another bone stock trans out of a fresh kill Aero.
MY ADVICE: the cost is slightly less to buy a rebuilt trans and install it, vs, cracking your trans open to fix the faulty gear, but I'd go the distance and have your trans evaluated and upgraded with a Quaife LSD. That way you won't have to worry about it in the near future.
