I am baffled by this after installing new pads and rotors. Initially, only the outer inch or so of rotor was getting swept by the pad, now it seems to be more normalized. The uneven wear led to uneven depositing of brake material on the rotor, causing bumpy brake action (a.k.a "warped rotors"), but some agressive break-in braking seems to have cured most of that. The calipers were replaced with NEW OEM units by the previous owner about 80,000 miles ago.
