Dangerous Steering Problem/Mystery - need help!

This one has me stumped and I've done a lot of work on this car so I'm pretty familiar with the mechanics of it.
Symptoms are that during acceleration/decel the car will jerk to the right or left and sometimes during higher speed turning (steering under load). I can't say if it is just one wheel or both, but when accelerating the steering will jerk left (with steering wheel off 10 to 20 degrees) and then under decelleration will jerk right (with steering wheel aligned OK). This is almost an immediate jerk (hence the dangerous situation) which requires an immediate steering correction. It has been progressively worse over that last 6 months. It is a 91 9000 turbo with 200k miles.
Here is what I know it is not:
Control arm bushings - new
Ball joints - new
Strut bearings - checked, seem OK
Swaybar linkage/bushings - new.
Tie rod ends - new in last 12 mo, 8000mi. Seem to be tight.
Steering column pinch bolt - tight.
I used a crobar to pry the steering rack, and the bushings 'seem' to be OK as I wasn't able to get much movement. Also I'm not getting any groaning or hard resistance from the steering rack so I 'presume' it is also OK.
I am starting to get some mild clicking from the right CV joint (the cheap driveshaft replacements from eeuroparts), but I can't see how a driveshaft could cause these symptoms.
Are these symptoms of inner tie rod failure? I hear these are very rare and they have not been replaced. Is there a way to check other than pulling back the boot for visual inspection?What to look for?. Last time I had the car up I tried shaking, pulling etc the spindle but couldn't find anything unusual.
Getting a bit desperate, and thinking for switching the whole rack out (though I know the rack itself is working fine).
Any help would be appreciated.
