5 speed rebuilt 2500 mi ago-MAJOR LEAK

1994 9k Aero, 180,000. I had the transmission rebuilt in August, put about 2500 miles on it, not a drip, felt great. All of a sudden yesterday I was driving down the highway and noticed smoke in the mirror. I pulled over and there was tranny fluid everywhere, running out on the ground until it was almost empty. Got it towed home. Looked underneath it and noticed the three bolts buy the spacer on the bottom of the transmission(by the cat. conv.) were pretty loose. I tightened them up, drained what was left, not much at all, and refilled. Went for a ride around the block and there was a trail behind me. Got it in the garage and it was just running out again, almost emptied on the garage floor again. One of those 3 bolts was still a little loose. Going to try this all over again, but was that 1 sort of loose bolt my problem, letting all the fluid out as quick as it does, or is there a bigger issue. Right now it is very hard to tell where it is coming from. Almost looks like something is missing by the spacer piece on the bottom, on the "other side" of the spacer. Any ideas where to start this time?
