Study holes in any wheel that comes loose.

These need to have four holes with the correct conical taper seat for the bolt heads, also an unmarred center hole that fits on the center rim on the hub.

Only one bolt plus the center rim is necessary to precisely locate them, or two, but four in perfect shape is the norm and what you want.

If a wheel has run any length of time loose, it will hammer on the holes. If they are out of round or the tapers are spoiled, you need to get rid of that wheel and get another.

It will never tighten correctly, no matter the torque you put on the bolts, and it will tend to work itself loose from not sitting centered on the hub.
This is obviously a safety concern, for yourself as well as anyone else or any other vehicle that a loose rolling wheel collides with.

Anyone with wheels that tend to work loose should study the holes. I doubt they can be repaired.
