I'm with you, no failures. In stock configuration the upper bolt is supported by the curved bracket. There must be some unintended loads on the upper bolt for it to fail. Something is loose allowing shock loads on the bolt, or the belt is lumpy causing a lot of tensioner activity, or the tensioner is sticking causing undue loads on the bolt.
Are there any other signs of wear in the tensioner assembly and mounting? It would be nice to see what the failed bolt fracture(s) looked like. These tensioners have a lot of miles on them with only a few breaking the bolt. Something has to be wrong with those few. Sure be nice to put a couple of strain gauges on his bracket to get an idea what is causing the highest loads.
