Any vehicle with sequential fuel injection/spark has to have cam reference data. Our vehicles obtain it by looking at where combustion occurred after sparking both of the cylinders which are at TDC.
The rest of them out there, save for the 1.9L saturns and probably a few others, do indeed use cam position sensors even if they don't have variable cam phasing. Those with a DOHC setup and variable cam phasing on both cams usually use two sensors, one for each cam (yep, a DOHC V-engine with variable cam phasing on both cams usually has 4 cam position sensors).
As far as David's P0340, it's either a DI, plug, or wiring issue. Throw some new NGKs in it, gapped correctly, and see what happens.
