I have a hard time imagining how a fuel filter could make you run rich, I think we can probably ignore that safely for now.
A broken vacuum line to the Fuel Pressure Regulator, or indeed the regulator itself, could make it run rich. Otherwise, in a non turbo, I have a hard time seeing how an air leak anywhere could make it run anything but lean.
That leaves us with o2 sensor and clogged exhaust, which are both worth evaluating.
The NTC sensor (engine management's coolant sensor) is one thing thats probably suspect and you didn't mention.
Have you though to do a compression test?
On the other hand, maybe your o2 sensor made the ecu adapt to a lean idle you introduced with the broken PCV system, in which case what you need to do is fix the PCV leak, and leave your battery unplugged over night.
