I have removed the whole pedal assembly (and specifically the "5th" bolt) without removing the dash top/instrument panel. Remove the bottom piece of the "clam shell" that surrounds the steering column. Look in immediately underneath the steering column and you will see the round cut out hole that Jeff refers to and the head of the bolt which is accessible with the right set of extensions.
Much easier this way.
I did it by myself, but Jeff is absolutely right about the need to pull the master cylinder forward (into the engine compartment). Otherwise the pedal assembly gets hung up on the four studs on the back of the master cylinder.
If I were you I would try leaving the pedal attached to the throttle cable as removal is a real chore.
