Over Charge Voltage

Dear board members,
My 87 9000 turbo is acting wierd for my father. The battery slowly drains over time. Battery replaced this winter. We checked voltage across battery, 12.0 volts. Charger was pumping in 5-6 amps on charger gauge. Following over night charge, battery reads 12.0 volts no amp draw to charge. Start car voltage on dash readout is 10.9. drive several miles back in drive way voltage reads 16.8 on dash pictogram. Battery shows signs of boil over, precipitate on battery side and coming from overflow port on battery.
Is it possible battery is overcharging? What is first thing to check? I cleaned the positive connections prior to battery terminal, they seem okay. The alternator was replaced approx. 2 years ago and there did not seem to be a problem immediately after replacement, but my father indicates that the charging system does not seem to be functioning as he has had to go out and charge the battery on a regular basis. It runs after full charging but eventually will loose charge after several weeks of short trips.
