Nevermind, I saw an old thread [below] that answers some of the questions.
The answers are: Heat shield is necessary. Above, to avoid burning the carpet and other things. Preferably also below, presumably to avoid igniting the grass. And, aftermarket converters usually come with heat shields, both above and below.
So, I guess the stock converter on my car can set off brush fires, as there doesn't seem to be a bottom shield. I think I can make out a top shield in the pic below, but that could just be my imagination.
