Diagnosis should be some minimal amount - whatever the minimum charge the shop has - maybe a half-hour. The upper and forward mounts can be seen with the hood open and a flashlight. Excessive engine travel can be diagnosed in about 2 minutes with a floor jack. I would expect the diagnosis to be included in the price if they do the work.
Price can vary widely. I believe the list price for the two lower mounts is about $160 each for parts. I think my Saab dealer quoted about $500 to replace the mounts, parts and labor - that works out to about 2-3 hours of labor. A local independent (trusted) shop quoted me about $350, parts and labor. Most of that was a parts charge of about $120 each.
I've bought the parts mail order for about $80 each. (total of $160). I would have had my local shop do the labor, but they insist on using 'their' parts, at their prices. So I did the job myself. If you can find a shop that will use parts you provide, expect around 2 hours of labor or so.
That's for the lower mounts. For the upper mount (dogbone), I really don't know how much the bushing cost from the dealer. I bought them mail order (Sobstory) for $23 each. You can pay a shop to press the old ones out and press the new ones in - that should only be an hour or so of labor.
Do you have a manual or automatic tranny? The diagnosis is pretty simple if you have an automatic. Just open the hood, and get in and start the car. With the brakes on, shift from Park to Reverse to Drive, while you look at the top of the engine (you should be able to see it with the hood open). If the top of the engine moves more than about 3/4 inch, and if it moves a lot then kind of 'bounces' when you shift from drive to neutral and back, then the mounts are probably bad. Inspect the bushings in the top 'dogbone'. A few small cracks in the rubber is normal. But if there are cracks entirely through the rubber, the bushing needs replacement.
