I have heard that of cases where it failed without warning, but in my experience it gives plenty of warning. When they've failed on me, I knew about it for a week. It usually starts as a short squeal on cold startup. At each succeeding cold startup, the squeal lasts longer. It's not a squeal that you can easily miss.
My approach would be to monitor it, but don't replace just in case it may fail. If you end up having to replace a main seal or oil pump o-ring, the pulley will be off, and you could think about replacing then. But I would leave it be. I've run two cars in excess of 150K with no balancer failure.
