On my 2.3i the exhaust sprocket was worn, chain had stretched, tensioner was all the way out. As a result I would occasionally hear a rat-tat-tat during cold start and depending how many teeth the sprockets skipped I could get a very strong vibration starting sharply at 2800 rpm. I replaced the chain and the worn gear and got rid of the problem.
I still sometimes on certain temeratures hear a brief rat-tat-tat for a few secs which I think is the chain slapping against the tensioner when the oil pressur is not up yet (I hope someone could second this!).
I have not experienced roughness. There is a design fault or then the job wasn't done right last time because it has been done twice in 200kkm!
