My '91 9kt lost a chain at 65k, but the technician only replaced the timing chain and the bent valves. Based on threads from the bulletin board, I decided to replace the Balance Shaft gears, chains, guides and related timing chain components at 115k. I found what I consider to have been a significant amount of wear on the BS sprockets, one in particular was heavily "scalloped" with teeth that appeared thin and brittle. It is hard for me to estimate how long the gear would have lasted, but I am glad I did the repair, if only for my peace of mind. I also found some deep groves in all of my chain guides, especially the timing chain. I wondered how this impacted the tensioner's ability to maintian proper tension.
Anyway, as a related aside, there was an alarming post regarding an individual who had a dealership replace all BS and TC components, but as the tech re-installed the TC cover to the block, he inadvertantly knocked off the Oil pick-up O-ring, causing the engine to run with low oil pressure and ultimately fail. The point of this frightening tale is to make certain that you select an experienced and reputable tech do the job. I was very careful and lucky that I did't make a similar error!
