I replaced all of the timing and balance shaft chain components on my '91 9kt with 115k as a precaution. The BS gear was worn quite a bit, in my opinion. The guides were well worn as well. To answer your question, I can't see any way that you could evaluate the condition of the BS components from above. Too many quides in the way. To answer your other question, it is my understanding that the teeth on the BS gear become worn to the point that they break off and go through the lubricating system, taking out the engines bearings. Sorry to be bring up such a downer, but I believe this to be true. This is far worse than a broken chain and the ensuing bent valves. . .
