Hmmmmm... how often do you adjust the clutch freeplay? Do you really have a problem with the clutch hydraulics?? You better check the fluid levels and see if it has dropped.
For the clutch itself, its time to replace when it slips. One check is to try to start off in a higher gear and see if it slips. Or accelerate in a higher gear as you drive up a hill- basically floor it and see if it slips. You can still drive for a while with a slipping clutch. What you DON'T want to do is go to the point where the rivets are digging into the flywheel- you'll hear it if you go that far. At that point, you'll do damage to the flywheel and it will need resurfacing as a minimum.
