Tranny teardown... phase 2 - got questions...

Ok, so I've gotten everything off except for the intermediate driveshaft. I was able to get the ball joints out with a special forked pry bar made especially for ball joints - unfortunately both rubber boots were trashed in the process. Thankfully those can be replaced (anyone have the part number and a place to purhase them? I don't see them on eeuroparts).
So close... decided to take a break for today - pros could have certainly gotten the tranny out by now... but I ain't a pro.
How do I remove the intermediate drive shaft from the tranny? The Haynes manual and the tutorial I've been looking at don't have a clear procedure - and I don't want to start removing bolts when I don't know what's holding what in place you know?
Also - as far as I can see, there are four bolts (16-18 mm I believe) and one nut holding the tranny in place. Am I missing any? Any tips on the next step - taking off the tranny?
