Its called triangulation, using multiple cell sites to pin down a location. You are correct that it would take more to pin down an exact location, but it would be good to a few hundred feet. And no you phone is not completely off when you turn it off, there is still a carrier signal transmitted back and forth between the phone and the cell site.
I spent 15+ years in the military working Communication security and Emission security. I’m just saying it can be done, but the point is that unless someone has a reason to do it, its not worth the time, effort and equipment.
Onstar is a separate issue; it uses GPS tracking and is much easier to track. That’s how they can tell where you car is if it is stolen.
