The silencer is inside of a pipe, it cannot fall off.
I think that you might be hearing the upper idler wheel failing. When it goes, and they all will fail if not changed out first, you dump the serp belt and loose the PS, alternator, A/C, coolant pump. You cannot drive as the engine has no coolant circulation and will overheat. You then get towed.
The pully is easy to inspect and replace if you are into DIY work. Good time to do the short belt mod at the same time.
Some other things driven by the belt can make noise when they are failing, but those do not happen very often. The pullies are a known issue and that is where you should look first. If the pully is good when inspected, you have the belt slack already. Turn the other driven accessories by hand and feel for any radial or axial play or roughness. You are doing these things with a cool engine. With the belt clear of the crankshaft pully, start the engine and listen for sounds. Anything that you hear will be just the engine. Do not idle for very long as the coolant pump is not working. You do not want the engine hot as that makes for unpleasant work getting the work completed.
