cause and effect: what caused chain to fail

Chains just do not snap, there is probably a lubrication failure, perhaps caused by sludging and that can be created by using the wrong oil or neglected oil changes. And loose head bolts may play into this some how.
Balance chains often are the first to fail and if that chain gets loose, that can jamb up the timing chain. You may be in for new timing and balance chains, chain guides, spockets etc. Untill the timing cover is off, the full extent of the damage is really not known.
If oil changes neglected or inferior oil, piston rings may also be stuck. Damage to the piston crowns might be more than cosmetic.
With the valve cover off, evidence of sludging should be evident if it exists, also fish for solids through the drain plug. And the oil pan perhaps should be pulled to look for bits of chain or sprocket teeth, and the oil pickup screen cleaned. The oil pressure relief valve should also be dismantled and checked for sticking, scoring and rough spots smoothed out and perhaps the relief valve piston replaced if too damaged.
