The bottom idler pulley doesn't put any extra tension on the belt. The moving tensioner pulley is the only one creating more or less tension depending on where you have matched up the two lines on tensioner pulley.
The lower pulley is used to time all 4 cam shafts to crank shaft.
The middle pulley is used to time 1-2 camshafts to 3-4 cam shafts.
