Good question. Technically speaking, if the engine is at speed and the timing belt breaks, i.e. the valve train stops and the crank continues moving (each centrifigally charged) then one could expect there is going to be some initial and most likely repeated contact with valves and piston tops.... Which pistons and which valves you ask... That's for your mech to decide... If your mech checks each valve (there are 4 for eacy cyl.) and determines which are damaged, there might be some possibility that he misses one or more that are just slightly scathed.. The engine might run okay, but that would harm some performance and potentially ruin something down the line.
All that being said, I've seen some engines go through the dreaded valve-piston tango and get by well with replacing a few valves (no pistons) for thousands of trouble-free miles.. and some that just didn't go at all..
Clear as mud, eh!
