For the scenario you mentioned, you'd need to be in an accident serious enough that your car is disabled, serious enough that you can't call for help yourself, but not so serious that you can't talk to a dispatcher. What do you think the odds are of that?
If you cut out the GM marketed hype about talking to the dispatcher after the accident and assume you are unconscious, the on-star folks might be able to send help based on the crash notification - that of course, assumes that this serious accident has taken place in a remote location where 100 people driving by with cell phones have not already called it in. Calculate the odds again.
On-star is cleverly packaged, overpriced insurnace marketed using a fear factor - don't fall for it.
