Grinding, squealing Brake

A had the brake pads on my '97 900S replaced a couple of weeks ago (mechanic said rotors looked ok). In the last few days the brakes on one side have developed a squealing followed by a grinding noise when the brakes are applied. The brakes also chirp a bit after the brake is released - sounds like the pad is dragging. I notice that the rotor on the back on the side with the noise is badly scored - there is a groove about thumb width dug into it. Could some screwup in installing the pads lead to this? I'm heading back to the mechanic and some feedback beforehand would be helpful. Thanks.
