weird brake rotor wear pattern

My passenger side rear brake rotor has a weird wear pattern. The first half inch from the center isn't getting hit by the pad (expected), then about an inch is grabbing. Finally, the outer 3/4 of an inch of the rotor are completely rusted as if they haven't been in contact with the pad ever. I plan on taking the assembly apart next weekend (I've been driving it like this for months now without noticing).
What should I expect? Could this be as simple as a faulty pad? Or should I be expecting something a little more dire like a semi frozen caliper? I guess I can take a picture of the wear pattern if necessary.
Another point, because the rotor has been wearing basically this track in the center for so long, do I need to put a brand new rotor on there? Can I just take a wheel and clean it up real good?
