This actually happened twice to me. I was able to take the retractor cover off to find that the spring mechanism that holds a small latch open broke, therefore making it impossible to pull out the belt. For the interim I left the trim panel and the retractor cover off and stuck my finger in the hole to release the latch from the retractor wheel while pulling out the belt. Be careful, I pinched my finger once doing it. Also make sure that you plug the pretensioner back in or the airbag light will come on.
The first time I was going around a sharp curve; replaced it with a used seatbelt unit. The second time I went over a speed bump too hard; replaced that one with a new unit where the dealer told me this is not that unusual. So, yes slamming the door could cause a worn out unit to fail.
