Never actually noticed which shoe moves, but that's not important right now.
When you spin a wheel, you should hear a little friction from the shoes.
The rotational uneveness is more likely the drum out of round. Take it to a decent crake place and have them re-turned. When complete, you want a smooth surface, too. Sometimes they do things too fast and set it up too aggressively and you end up with a rough looking surface.
There is really no excuse for turning out of round, though. Had it happened once... pretty irritating.
As for the adjustments, the star thingy will get you close. They're designed to self adjust when applying the brakes while travelling in reverse.
Wouldn't get too caught up on uneven wear, as long as you got good pad and good pressure, it should even out to get you stopped.
