violent shaking with clutch engaged

I have a 1999 9-3 SE, manual trans 5-speed, 143K mi.
There is a violent shaking I can feel in the steering wheel, whole car and especially the gear shift at all speeds and all gears. However, when I push in the clutch pedal at any speed and coast or even depress the accelerator with the clutch pedal in, the car is as smooth as can be. If I coast with the clutch pedal out, the shaking is present, but not quite as severely as upon acceleration. Also there is a clunk from the engine if I suddenly take my foot off of the accelerator with the clutch out. In neutral with the clutch out, there is no shaking.
I have:
Checked the wheels for balance
Regapped the spark plugs to 0.9
Checked transmission mount - looks ok, but there is play in the rubber gasket that holds it to the brace.
Checked the drive axels for play. Both RH (passenger) and LH (drivers side) have about 2 mm of movement side to side in the companion flange, but just the RH rotates about 2 mm in the flange.
Has anyone had a similar problem? I would appreciate any help or ideas on this. TIA.
