I have not had my rear bumper off, but it's probably like the front bumper. There are two giant poly brackets that are attached to the bumper itself, and those fit into some metal tabs on the body. If the tabs are bent, the bumper will fit incorrectly. They are easy to adjust, you can bend them with your hands or flat pliers. I would wager that if your bumper has been painted that there was a rear end accident and these are out of line.
As I mentioned, I have not had the bumper off but I think you just remove a couple of bolts and then pull it to the rear to get it off. You might have to unsnap the side clips by pulling the sides out a bit. I don't think you can break those clips or the bumper using your hands, so don't be shy. I have some notes somewhere I can dig out if no one responds with more specific instructions.
