Well, bullet proof does not really mean 100% non failures in all cars produced and rolling on the roads.
Have you had the car since brand new so you know the history of all fluid changes in transmission? I assume the fluid level is right?
These 4 speed autos are very sturdy and I don't think I have replaced one in a 900 or 9-3 yet but a couple in the 9-5 and maybe 3 or 4 in the Volvo 850/70 through all the years so I would certainly say they are bullet proof.
It might be some o-rings getting tired inside allowing fluid to drain back but it would take a teardown and rebuild to cure it.
Try maybe with a can of TransX additive and see if it restores some of it.
