I don't remember any structure that was embedded in the foam, but I never actually removed the foam. There was a main block of foam (shaped to make up the seat) and the heater grid was on top of that. Above the heater grid was a thin layer of foam, seemed like it might have been a different material.
Seems to me I had to peel away that thin layer, then repair the heater & place it back. I probably tried to glue it again, but don't remember what I used. It was quite a few years ago.
In my case, the wire had broken just outside the spade-connector at the little button temperature-switch. I tried a bit of mechanical reinforcement but I'm not sure that made any difference.
