remove the caliper and the carrier so that all that's left is the rotor and screw the bolts in through the holes where they go it to hold the caliper. Keep tightening and eventually they will hit the rotor from behind (which will ruin the rotor, but if you are going to toss it then you probably don't care. Otherwise, put a thin piece of wood between the caliper and where the bolts will make contact). Keep tightening until enough force is exerted to pop the rotor off, or break the rust on that side.
You might have to loosen, turn the rotor and repeat. This is much safer than hitting the rotor with a hammer or mallet, which could potentially damage something. Furthermore, doing that while the car is on jackstands is definitely not advisable.
