The timing chain can be replaced in the car in one of two ways. You can roll in a new chain with a master link, and then crimp it shut, or you can unbolt the engine mounts, slide the engine over on a pair of trolley jacks, and take the timing cover off. The timing cover has to come off if you have cracked, broken, or heavily scored guides that need to be replaced, or if you want or need to replace the balance chain. You can take out the tensioner and measure its extension to get somewhat of an idea of how your chain is. If you search the BB, you can find the maximum allowable length.
