Did removing the tensioner work?
The main problem you are having reinstalling the sprockets is the fact that some valves are always depressed some degree, and that causes the cam to want to settle in a rest position where the forces are equal. This is NOT necessarily where all your sprockets will line up.
What you need is a big open-end wrench to grab the cast hex on the camshaft, and use that to rotate the cam slightly so the key on the sprocket goes into the slot on the cam. When the bolt is torqued down, all the bits will be where they are supposed to be. the forces from any partially depressed valves will be transferred to the chain.
As for the machining, the cam is machined new to a specific size. The head/cam carrier is cast to a specific shape, as are the caps for the cam bearings. The head/cam carriers are machined flat to a dimension, and the caps are as well. The caps are installed on the head in their proper positions, and then the bearing galleries are machined to a specific dimension by a long bore tool that machines all bearing positions at once. This ensures the cam galleries are straight, even if the head is slightly out of dimension.
