Wheel Balancing follow-up/problems

Hi all,
Well, I took the car back to the place that originally mounted my new 16's with my new bridgestones, and they rebalanced them. Turns out one of the wheels had thrown a weight. However, after I got the car back, it still shakes above 60mph. Now, I trust this mechanic, mainly because he was willing to drive me to work from the shop in the morning, and pick me up when my car was ready personally. This is good service, especially since it was warranty work, and it's a firestone chain store. Plus, I have had him mount and balance 3 sets of tires before this one and they have all been flawless. So, this begs the following questions:
1) Is it possible to have a tire be balanced, but out of round or something so that the machine doesnt pick it up?
2) What is needed to determine if a rim or tire is out of round? If it is a problem with the tire, i need to send it back to the tirerack, if it's a rim, then it needs to go back to sportscarservice.
3) Could there be something wrong with a part of the suspension that is causing this vibration? Maybe going from a 15 to a 16 inch rim with lower profile and wider tires makes a problem more pronounced?
Maybe the firestone wheel balancing machine just sucks, but then again, he has had no problem doing this work for me in the past. Please advise on what I should do next. It was very frustrating to have to put my 15's with snow tires back on the car to drive down to NC this weekend.
