I've been fighting that as well. The time that it didn't come unstuck, I pulled the trim panels off to see how complicated replacing it was going to be and saw that the helical spring which gives the adjuster it's "push" upwards had gotten stuck around the bottom of the adjuster plate and was pinching the seatbelt. The fix was simply to push the end of the spring back in it's pocket on the adjuster plate.
The panels are held in place with a combination of clips and screws, remove them carefully. If you can wait until tomorrow, I'll scan the appropriate pages and email them to you.
