The only seriously rusty exhaust work I've done has been on motorcycles; my cars have been pretty easy in comparison.
I assume you've gotten the clamps off already... if not, use liquid wrench or the like on the bolts, and let it soak in overnight.
On my 30 year-old bike, I needed some force and a blowtorch to remove the original mufflers. I heated the frozen joint (the liquid wrench I had previously applied lit on fire now... be careful), and then used a 5-lb mallet to drive a log to knock the muffler off. Sometimes you can just heat and cool the joint repeatedly with a torch, (the metal expands and contracts, which cracks some rust off) and have luck bashing it off from there (you're not keeping the muffler, right?).
Sometimes exhaust work just isn't pretty...
