I learned to suspect that more than one thing can be going wrong with the NG setup at the same time. I've been ready to write off the box many times now, but it's still in there...
First time was the old style linkage. Granted, it was the well-known bushing issue, but when I upgraded it, the old style also showed an alarming amount of wear for a (then) 5-year-old part. Vast improvement given each linkage in working order. (I don't rememebr if you did this already...)
Second time was general grinding, after trying MTL (ground incessantly) and a few other things, I used a heavier Mobil1 gear oil, which quieted down everything but 3rd, and even that was occasional. I wanted to try MT90 but I haven't driven the car much lately.
Third was the input shaft splines. The case has a port to inspect the clutch, which renders the case essentially open and those splines load up with junk. It causes more of a hard-clutching problem, but it can also make the clutch act inconsistently. I cleaned them from the outside with some wd-40 and tried to lube them up.
Not to mention clutch cable, good old fashioned worn clutch, etc., but it doesn't sound like those.
Mine still very rarely leaps out of gear but the grinds and vagueness are gone. My passenger side mount in mine is bad, but it does pass the lurch test, so it doesn't 'seem' broken.
I think at this point, a reman box may be cheaper than/equal to tearing this one down, so you may want to look around for those and drive this one until it falls out. Good luck, man...
