The red DI cassettes (t-5) have a variable mileage range before is dies. Be thankful it is not the Black DI in the newer cars as they are prone to failure earlier than the red ones.
Question #2 - If it was part of the scheduled maintenence, people would flip out adding 300-500 bucks to their bill for a "scheduled Maintenence item. The lifespan ranges greatly that maost people would skip that. With a spare in your trunk, you are prepared when it does happen, but putting one in prematurely could be more costly than replacing it when it dies. I'd opt for the spare in the trunk idea.
All are prone to fail at some point, but the red DI's can last over 80K, yet some go much earlier. Get a spare, and ease your mind. Spending 300 aftermarket from eeuroparts for picking one up on ebay might save a few bucks.
And Yes, everyone without a spare DI in their trunk is living on the edge.
