You have the right idea going to avoid dams: Keep the deck cold to avoid melting. Good ventilation is the key factor, but it sounds like you've done what you could. How is the ridge vented? (is a continuous ridge vent, hats, or gable end vents?). A lot of the heat in a attic is from solar radiant instead of heated space below assuming you have good insulation down at the ceiling. If the ventilation isn't good, the heat builds up and will slowly release it when things start cooling down after dusk which creates that water hitting the cold eave and freezing.
Just guessing here, but you are probably not going to fix it easily. R-30 requires 10" batt insulation. You need 2" min clear to allow for ventilation at the eaves. 12" clear is needed plus you need ventilation at every joist pocket and good ventilation at the ridge. Assuming you are trussed, and those truss top chord comes right down to the wall plate, you don't have enough room to vent and insulate properly. If you have the space, your idea might work if you can ventilate the attic properly.
Vapor barriers have very little to do with ice dams...
The best method is building a true cold roof (basically a double roof with ventilation running up between both roofs), but it's not inexpensive.
