Many have trouble absorbing B12. (Very few do not do well with the sublingual tablets either. B12 injectables can be had in the USA by simply asking for them. So that is a good solution for those who bother to learn how to do injections and do not want to visit the doctor's office every two weeks.)
I don't know if the Swedes have a particular problem with B12, but they were very progressive in switching from injected to sublingual and the publications about this may have created the impression that there was a Swedish problem. They have universal health care, so one would expect to see things like this been highly utilized. So more use in Sweden might be contrued as an indication of a problem when in fact it might represent a better response to a universal problem.
Some eldery who are frail and fading who cannot walk, are able to after B12 shots. That was the case with my uncle.
